How to Save Money on NHS Prescriptions
PPCs, exemptions, HRT PPC, and other ways to cut the cost of NHS prescriptions in England.

Check if you're already exempt
Before paying for a PPC, check whether you already qualify for free NHS prescriptions in England.
A wide range of exemptions cover free NHS prescriptions in England, including anyone 60 or over, under 16, aged 16-18 in full-time education, pregnant or within 12 months of giving birth with a valid maternity exemption certificate, people with certain long-term medical conditions holding a medical exemption certificate, and those on qualifying benefits or the NHS Low Income Scheme. If none apply, a Prescription Prepayment Certificate is the next thing to consider.
Work out if a PPC saves you money
A Prescription Prepayment Certificate (PPC) lets you pay a flat fee to cover all your NHS prescriptions for a set period, regardless of how many items you need. It only pays off once you cross a certain number of items.
| Payment route | Cost | Best for |
|---|---|---|
| Pay per item | £9.90 / item | 3 or fewer items in 3 months |
| 3-month PPC | £32.05 | 4+ items in 3 months |
| 12-month PPC | £114.50 | 12+ items across a year |
| HRT PPC | £19.80 | Anyone on repeat HRT prescriptions |

The HRT Prepayment Certificate
If you're prescribed Hormone Replacement Therapy, a separate HRT PPC costs £19.80 and covers all your HRT items for 12 months, regardless of how many prescriptions or how often you collect them — it's specifically for HRT and separate from the standard PPC.
Other ways to cut costs
- Ask your GP if a 3-month prescription is clinically appropriate instead of monthly, reducing the number of separate charges
- Check whether a generic version of your medicine is available and suitable — ask your pharmacist
- Use Pharmacy First for the 7 qualifying conditions instead of paying privately or visiting a GP
- Compare private service prices between pharmacies for anything not covered by the NHS, like travel vaccines or flu jabs
- Buy a PPC by direct debit for the cheapest ongoing rate if you know you'll need it long-term

Frequently Asked Questions
How many items do I need for a PPC to be worth it?▼
A 3-month PPC (£32.05) breaks even at 4 items in 3 months. A 12-month PPC (£114.50) breaks even at 12 items across the year — so if you take one or more regular prescription items a month, it's almost always worth it.
Can I get a refund if I buy a PPC and then become exempt?▼
Yes, you can usually get a refund for the unused portion if your circumstances change — contact the NHS Business Services Authority to check the current process.
Do I need a PPC in Scotland, Wales, or Northern Ireland?▼
No — prescriptions are free for everyone in those nations, so a PPC only applies if you're paying the England prescription charge.
Figures are published NHS rates correct as of July 2026 and are for guidance only, not financial advice. NHS prescriptions are free in Scotland, Wales, and Northern Ireland — these savings only apply to prescriptions collected in England.
