Get assessed and treated for 7 common conditions at participating pharmacies in England without a GP appointment — free on the NHS.
NHS Pharmacy First launched in England on 31 January 2024. A trained pharmacist can assess seven common conditions in a private consultation area and, if the clinical pathway allows, supply certain prescription-only medicines such as antibiotics — without a GP appointment. The service is free on the NHS at participating English pharmacies.
This page is a directory of pharmacies that list Pharmacy First among their NHS services. It is not a diagnosis, not a booking system, and not a guarantee that a given pharmacy can treat you today. Age bands, pregnancy, red-flag symptoms, and local stock all change what the pharmacist can do. If you are very unwell, pregnant with a suspected UTI, or the pharmacist says you need a GP or urgent care, follow that advice.
Which seven conditions does Pharmacy First cover in England?
England’s clinical pathways cover sinusitis (usually aged 12+), sore throat (5+), acute otitis media / earache (children 1–17), infected insect bites (1+), impetigo (1+), shingles (18+), and uncomplicated lower urinary tract infection in non-pregnant women aged 16–64. Each pathway has its own age band and exclusion rules. The pharmacist examines you, uses the published scoring tools where required (for example FeverPAIN or CENTOR for sore throat, otoscope for earache), and decides whether supply is appropriate.
If you fall outside the age band, are pregnant, have red-flag symptoms, or the infection looks complicated, the pharmacist will not complete the pathway as a substitute for a GP. That is the service working as designed — not a refusal of care.
Do you need an appointment, and what does it cost?
You do not need a GP appointment. Walk into a participating pharmacy, or you may be referred by your GP practice or NHS 111. Many pharmacies still prefer a booked slot so a pharmacist and a consultation room are free — calling ahead avoids a wasted trip. The consultation and any medicine supplied on the pathway are free on the NHS in England. You are not paying a private prescription fee for Pharmacy First supply.
Who this is not for: anyone who wants antibiotics without an assessment, anyone shopping for a private prescription, or anyone whose symptoms need urgent or emergency care. Pharmacy First is a minor-ailment pathway, not A&E.
Is Pharmacy First the same in Scotland, Wales, and Northern Ireland?
No. Scotland has a separate, longer-running NHS Pharmacy First Scotland scheme that covers a wider list of common ailments than England’s seven pathways. Wales and Northern Ireland run their own common-ailment services under different names and rules. If you are not in England, ask the pharmacy in front of you what it can do today — do not assume the England age bands apply.
This directory’s Pharmacy First flag is taken from NHS service data for English community pharmacies. It does not mean every listed premises can treat every pathway on every shift.

Is Pharmacy First available in Scotland, Wales, and Northern Ireland?▼
Scotland has its own NHS Pharmacy First Scotland scheme, which covers a wider range of conditions than the England service. Wales and Northern Ireland operate similar common ailment services under different names — ask your local pharmacy what is available.
Will the pharmacist always give antibiotics?▼
No. Many sore throats and sinus infections are viral. The pharmacist follows a clinical pathway and may advise self-care, a delayed prescription, or referral instead of immediate antibiotics.
Is Pharmacy First free?▼
Yes in England: the consultation and medicines supplied on the pathway are free on the NHS. It is not a private prescription service.
